Here is your chance for your Ashworth student-athletes to receive a free physical during the school day on April 26, 2024! This physical, since it will be dated after April 1, will be good through the end of the school year in May 2025. Please help us take advantage of this great opportunity given to our student-athletes by Advent Health.
The following are directions on the steps that need to be completed for our student athletes to be seen by Advent Health professionals:
1) Get a Physical Packet from Nurse Lee or Coach Poteet.
2) Students will have parents or guardians fill out, sign, and date the “Sports Physicals Parental Consent Form.”
3) Parents or guardians complete the “History Form”, which consists of pages 1-2 of the actual physical paperwork. They will need to make sure to complete all the questions (1-28) of the physical.
4) Parents or guardians will need to make sure they AND the student athlete sign and date page 2 of the physical.
5) The “Physical Examination Form”, pages 3-4 of the physical, will be filled out by the professional medical personnel of Advent Health.
6) Return the “Sports Physicals Parental Consent Form” and the Physical Form to Nurse Lee or Coach Poteet as soon as possible.
